Who we are
Patchwell is a product of Human Augmentation Technologies ("we", "us"). This page explains what we collect through patchwell.ai and what we do with it.
What we collect on this site
When you request a leak report, we collect what you type into the form: your name, practice name, email, phone number, practice type and practice software. We don't collect patient information through this site, and you shouldn't send any through the form.
Like most websites, our hosting provider records basic technical data such as IP address, browser type and pages visited, for security and to keep the site running.
How we use it
- To contact you about your request and set up a call.
- To prepare and deliver your leak report if you go ahead.
We don't sell your information or share it for advertising.
Patient data
We never touch patient records until your practice and Human Augmentation Technologies have signed a business associate agreement. Patient data is handled under that agreement and HIPAA, not under this page.
Your choices
You can ask us to see, correct or delete the information you gave us. California residents have additional rights under the California Consumer Privacy Act. To make a request, email us at [CONTACT EMAIL].
